About CEDE

The Center for Excellence in Diabetes Education (CEDE) is committed to helping people with diabetes improve their diabetes care and control.

The program teaches practical diabetes care and provides education and tools to help manage diabetes through exercise, nutrition, medications and follow-up care.

Mission Statement

Our mission is to provide ongoing information and education in diabetes self-manangement skills and lifestyle modifications to community members diagnosed with diabetes.  The overall goal is to promote improved quality of life for people with diabetes by improving outcomes and reducing risks of short and long-term health complications. In short, "fitting diabetes into patients' lives, not making their lives all about diabetes."

Classes for People Living with Diabetes

Insurance Information 

Our education program meets Medicare guidelines and is covered by most insurance carriers. Please check with your insurance company      for details of your coverage.  Co-pays, if applicable, will be collected at the time of the appointment. 


Scheduling an appointment:
Appointments for Diabetes Self Management Education (individual AND class) and for Medical Nutrition Therapy  require a Provider Order Sheet signed by your primary care provider or treating physician. 
You can find a copy of order sheet here.

You may also need an insurance referral.

We don't require the order sheet for appointments for medical management with the nurse practitioners.  However, your insurance may require pre-authorization from your physician.
