Diabetes Self-Management Education Program 

Patients entering our program will meet with a Certified Diabetes Educator (CDE) to review your current diabetes self management care. Depending on your needs, we will develop a plan together that could include group classes, medical nutrition therapy and individual follow-up education and support.

Diabetes Medical Management 

If requested by either you or your PCP, medical management of your diabetes can be scheduled with practitioners who offer short term consultation or ongoing management of diabetes. Diabetes management focuses on matching the diabetes plan to real life and providing support in adjusting to life with diabetes.

Medical Nutrition Therapy

Medical Nutrition Therapy involves nutrition assessment and counseling to help manage diabetes or other conditions.  The Registered Dietitian reviews your food and activity and works with you in setting goals to achieve better diabetes control. The emphasis is on making small, lasting changes rather than following a strict "diet". 

Insulin Pump Therapy 

We provide individual and group education as well as support for insulin pump therapy.
