How do I connect to the MyHealth@CooleyDickinson Health Record?
You will receive an e-mail from FollowMyHealth™ with a link when your health record connection is established. (The MyHealth@CooleyDickinson Health Record is provided by FollowMyHealth (FMH).) If you don't see an e-mail invitation from FollowMyHealth within 5 business days, please check your e-mail provider's spam folder as the e-mail may have been incorrectly flagged as spam.
Clicking on the link in the e-mail will bring you to a login screen where you will have two choices: "Create an Account” or "Add this Connection.” Select "Create an Account”. (Add This Connection is for users who have any existing Universal Health Record account with FollowMyHealth and wish to add Cooley Dickinson Hospital to their existing account.)
Once you have completed the first-time login, you will be able to access the MyHealth@CooleyDickinson Health Record by visiting www.cooley-dickinson.org and clicking on MyHealth in the upper right corner of the home page. If you have questions, call 1-413-582-2221 or e-mail email@example.com.
How do I log in?
You can create a new account using the FMH Secure Login, or you may use your existing Facebook, Gmail, Yahoo, or Windows Live ID accounts to log into the MyHealth@CooleyDickinson Health Record.
Although the portal uses your information contained in these accounts to authenticate your identity, your personal health information will never be made publically available on these sites.
What if I need help?
If you find you need assistance at any time during the set-up process, click on the Support link from the MyHealth@CooleyDickinson log in screen to our Support Team. We can be reached by e-mail firstname.lastname@example.org or by phone 413-582-2221.
Our Support team monitors phone calls and e-mails Monday through Friday from 8 am to 4:30 pm. Alternatively, you can click on the Help link from the MyHealth@CooleyDickinson login screen for 24/7 access to a general help page, which contains information and solutions to common questions.
To access the online help from inside the MyHealth@CooleyDickinson, click the down arrow next to "My Account” in the upper right corner of the screen and click on support. There you will find support contact information and links to the online User Manual, Video, and other resources.
What are the minimum technical requirements?
- HTML5 compliant browser (IE8+, Modern versions of Firefox, Chrome, or Safari)
- Supported on both Windows and Apple operating systems
- User can create a FollowMyHealth Secure Login or you may use an existing Windows Live ID, Hotmail, Google, Yahoo, or Facebook account to authenticate
- Iphone and Android Apps available from Itunes, Google Play, and Amazon App Store
Can I view and manage my child’s health record?
Although you cannot access your child's information on the MyHealth@CooleyDickinson Health Record, you can request paper copies by contacting the Health Information Management Department (Medical Records) at 413-582-2282.
Can I view and manage another adult's health record?

We are not presently offering Adult Proxy Access. However, access is available for adults age 18 and older who request access for themselves. Once an adult portal account through MyHealth@CooleyDickinson is setup, access can be given to others at the patient's discretion from within the account preferences. This is controlled and managed by the patient themselves.
