When You're Ready to Leave Partners in Care
Discharge

When your physician determines that you are ready to leave the hospital, a discharge order will be written. The hospital has a Case Management Department that can assist you with your discharge plans. The case manager will arrange community services, arrange for transportation, obtain home care equipment and coordinate any other discharge needs that you may have. If you have other questions following discharge, you may call the Case Management Department at (413) 582-2248.

Your primary nurse will provide you with instructions about your medication and care. Please collect your personal belongings and double check drawers, your bedside table and closet. Regular checkout time is 11:00 a.m.

Home Health Care

On admission, you will be given a list of home health agencies that are available in your area, including the VNA/Hospice Alliance, which is affiliated with Cooley Dickinson Hospital. If your physician determines that you will need home care services, a case manager will ask you to choose an agency from that list and will then coordinate the services for you.

Nursing Home Care

If you and your physician decide that a nursing home stay may be necessary for your continued recovery, a case manager will discuss with you which nursing home you would like to go to and then make the arrangements for you.

Case managers can also assist in explaining insurance coverage for the services that you may need after you are discharged from the hospital. They will also be in contact with your insurance provider on a regular basis to discuss what your care needs may be.

Ambulatory Care

Our 24-hour Emergency Department offers prompt emergency care, through a Fast Track program for minor medical emergencies. The department receives patients from all area emergency medical services teams, with over 30,000 visits each year.

Rehabilitation services include occupational therapy, physical therapy, speech and language therapy, as well as cardiopulmonary rehabilitation to inpatients and outpatients. Off-site locations are available in Amherst, Easthampton, Florence and South Deerfield.

Radiology services are offered by the Radiology Department, including routine and emergency consultation and evaluation such as x-rays, mammography, nuclear medicine, ultrasound, CT and MRI. Diagnostic radiology and mammography services are also offered in Amherst at 170 University Drive.

Laboratory services offered include routine and emergency laboratory testing of blood and other specimens collected at three locations in Northampton or at blood drawing stations in Amherst, Easthampton, Hadley and South Deerfield.

The hospital offers a fully licensed Blood Bank for people who wish to donate blood on a one-time basis or as a regular donor. For an appointment, call (413) 582-2162.

Behavioral Health

Our inpatient psychiatry service offers voluntary and involuntary hospitalizations for patients 16 years and older in an acute, short-term setting. An outpatient psychiatric service provides a partial hospitalization program and outpatient care at 10 Main Street in Florence, with outpatient counseling available at several satellite locations as well. The Massachusetts Prevention Center, also at 10 Main Street, provides a resource library and outpatient services for physical and mental health prevention concerns, substance abuse treatment and prevention, and compulsive gambling.

Inpatient Medical Care

Respiratory therapy offers diagnostic and therapeutic services for a broad range of pulmonary diseases and respiratory conditions.

Our Oncology Program provides coordination of inpatient and outpatient care, education, support groups and more for cancer patients and their families.

Surgery

Inpatient and outpatient surgery in a range of specialties is available at Cooley Dickinson. Our surgical day care unit meets the growing need for outpatient surgical procedures.

The Childbirth Center

Our award-winning Childbirth Center features six birthing rooms, private post-partum family suites and family-centered care, with deliveries by both obstetrician-gynecologists and certified nurse midwives. Over 900 babies are born at Cooley Dickinson each year.
