The Cancer Care Program at Cooley Dickinson Hospital is proud to offer the excellent, personalized service of its patient navigator program. This program, currently focused on breast health, is aimed at helping guide patients with a suspicious breast finding through the increasingly complex health care system to ensure timely diagnosis and treatment of their cancer.
Madelyn Breen, RN, CM, BSN, OCN
The Patient Navigator is able to:
• Guide patients through the diagnostic process
• Serve as a resource through the continuum of care
• Provide one-on-one emotional support
• Serve as a patient advocate
• Assist with coordination of care
• Assist with timely referrals
• Connect patients and families to resources for disease management
• Identify ongoing learning needs
• Provide education and educational materials
• Identify barriers to care
• Help access support services and support groups
• Direct patients to local resources
