Our Congestive Heart Failure (CHF) Program has been created to educate patients on how to successfully manage congestive heart failure.  Education is available to both inpatients and outpatients.  Our CHF Coordinator is available to provide resources to local skilled nursing facilities, as well as patients and families, to help reduce CHF hospitalizations and maximize quality of life for patients.
