As part of the Emergency Department, we offer case management services which are available from 8 a.m. to 6:30 p.m., 7 days a week. The Case Manager is a nurse who primarily tries to connect people to community resources such as visiting nurses, home health aides, homemakers, nursing homes and rehabilitation facilities as well as Highland Valley Elder services and Meals on Wheels. The Case Manager also attempts to:
- keep elders at home safely
- connect people with primary care physicians
- help people navigate confusing health insurance issues
- advocate for patient care and patient rights
- serve as a coordinator between patients families, staff, and physicians;
- make after-care arrangements and follow-up calls
