Thank you so much for your interest in volunteering at Cooley Dickinson Hospital. Joining our Summer Program gives you the opportunity to learn about the hospital, and help your community, and discover your own interests in healthcare. The opportunities vary widely, so be sure and check off your interests on the Openings form.

In order to begin the application process we will need the following:

• Completed Volunteer Application Form
• A copy of your Immunization history
• Signed Parent Consent Form
• A recent TB test
• Summer Openings Form

Please keep in mind the following:

• Mandatory Orientation: Thursday, June 26, 2008, 9:30am-12:30pm
• The 9 week Summer Program begins Monday, June 30 and ends August 29, 2008. You must be able to volunteer at least 7 of the 9 weeks, at least one 4 hour shift per week (many volunteers do more.)
• The deadline to get your application in is Friday, May 9, 2008
• A TB Test will be required if you have not had one in the previous year
• You must be at least 15 years old
